The Emergency Foster Care Wraparound Services Case Worker works within our Emergency Foster Care program to deliver collaborative planning, obtain treatment and support services, and manage resources that both the child and foster family need while a child is placed within an emergency foster care home. This position will report to the Supervisor of Emergency Foster Care who reports to the Director of Foster Care.
SPECIFIC DUTIES
- Assess the needs of the youth and family within an emergency foster care placement.
- Manage crisis situations.
- Work with all youth and families to provide any necessary resources to stabilize the placement and ensure the safety and wellbeing of the child.
- Refer and link youth and families to services or community opportunities.
- Assist with child and family team meetings.
- Advocate for youth and foster placements through community agencies, schools, and court systems.
- Collaborate with children, foster placements, and case managers to develop, support, and monitor the progress of treatment plans and interventions.
- Complete timely and accurate documentation.
- Follow all IL DCFS and Child Welfare code of ethics.
- Work under a Wraparound Services model.
- Follow a strengths-based, person-centered model.
- All other duties as assigned.
JOB QUALIFICATIONS
- Bachelor’s degree in a human services/social services field.
- Excellent written and oral communication skills.
- Strong crisis management skills.
- Demonstrated commitment to and understanding of Shelter’s mission, vision, and values.
- Must be CWEL certified
- Must have a valid Illinois driver’s license, automobile, and must submit proof of Illinois auto insurance.
- Must complete a Medical and T.B. test. Must be fully vaccinated against COVID-19, according to the latest CDC guidelines.
- Must be fingerprinted by the State of Illinois and must submit to a full Background Check through the Illinois Department of Children and Family Services.
- Must submit official educational transcripts from the last school in which a degree was awarded.
- Ability to lift 40 lbs
